D.C. New Student Enrollment Forms

If your My School DC lottery application has matched your child(ren) with LEARN D.C. Public Charter School, the next step is to complete your online school enrollment . 

There are three steps in the enrollment process. You must submit all required documents in the three steps outlined below and approved by the LEARN Enrollment Team before your seat is confirmed.

If you are not able to enroll online or wish to make an in-person enrollment appointment, please sign up for an in-person appointment below.

Enrolling by Email Instructions


Complete a Seat Acceptance Form via SignNow. You must complete a form for each child you are enrolling.

Complete the LEARN Registration Packet via SignNow. You must complete a packet for each child you are enrolling.


Also collect your documents and complete the DC Residency Documentation Checklist via SignNow. You can see the documents in the checklist below.

Our Enrollment Team will review documents and then send you a link to sign a Residency Verification via SignNow.


Once submitted via email, you will get a confirmation email that you have completed the process.

Documents you will need to Submit

Required Documents:

1. A copy of your child’s original Birth Certificate
2. A copy of the enrolling parent/guardian’s unexpired driver’s license or state ID
3. A copy of the child’s current, unexpired DOH Health Certificate and immunization record (Children will not be allowed to attend in-person instruction without current health and immunization documentation. If you have not done so, please contact your child’s pediatrician to make an appointment.)
4. A copy of the child’s D.C. Oral Health Assessment Form (due within 45 days from the start of school.)
5. Proof of DC residency

Optional Forms:
– Asthma Action Form (click here)
– Anaphylaxis Plan (click here)
– Medication Treatment Plan (click here)
– Most Recent Report Card, if applicable
– Individualized Education Plan (IEP), if applicable

For military-connected children only, we will also need:

– Verification that your child is an active duty military dependent: orders or a letter from a parent’s military command verifying the service member’s residential address in D.C.
